Requirements:

Board Eligible (to become Board Certified within 2 years of becoming Board Eligible) or Board Certified in Internal Medicine - [REQUIRED]
MI license (or willing to obtain) - [REQUIRED]
DEA (or willing to obtain) - [REQUIRED]
BLS, ACLS (or willing to obtain) - [REQUIRED]
Clean malpractice - [PREFERRED ] Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - [REQUIRED]
If candidate is TRULY Board Eligible, must become Board Certified within 2 years of becoming eligible
